Required Documents

Your child’s Admission Application is not considered complete until you have submitted all required documents to the school for your child.

After you have submitted your online Admissions Application, please call us at (718) 324-1088 to arrange for submission of the following:

• Birth Certificate
• Last Report Card or Progress Report for grades 1 to 8
• Standardized test scores for grades 1 to 8, when applicable.
• When applicable, the current Individualized Education Plan (IEP) on file with the Committee for Special Education (CSE) for all grades
• If applicable, 504 Recommendation for all grades.
• If Catholic, certificates of any/all sacraments received.
